I can obtain copies of Chicago death certificates, 1923-1947, at my local Family History Center on Tuesdays, Wednesdays, Thursdays, and Saturdays. Turnaround is quick, sometimes the same day, depending on when you submit the request, and the cost ($6) is less than you'd pay ordering a Family History Library film ($7.50) or getting the record from the county ($15).

* Illinois, Cook County Deaths, 1878-1922 (actual record images) are available for free on FamilySearch.
* Extracted information (no images) for Illinois, Deaths and Stillbirths, 1916-1947 is available at FamilySearch. I can get the corresponding images for Chicago records. Provider mollykennedy can get the corresponding images for any entry in the index.
* Chicago death certificates after 1947 are only available from the county or state. You can pay and download them from www.cookcountygenealogy.com.
* You can view a blank copy of the death certificate form used during those years from the link below. However, please remember that key information (like parent names) may not be there if the informant didn't know it.
* If I'm not able to find the record you need (it's rare, but it happens), I will let you know how and what I searched and there will be no charge except the $.50 Genlighten processing fee.
